چکیده

An 81-year-old man was referred with an infected cardiac resynchronization therapy defibrillator (CRT-D) system. His initial dualchamber implantable cardioverter-defibrillator (ICD) had been implanted 8 years earlier and upgraded to a CRT-D system 2 years before referral. Epicardial atrial and left ventricular (LV) leads were subsequently added after the endocardial LV lead lost capture. The endocardial LV lead was abandoned. One year before referral, a local revision was performed for impending erosion. Erosion recurred and methicillin-resistant staphylococcus aureus (MRSA) was grown. An attempted transvenous extraction 9 months before referral was aborted due to inability to

Acute stroke from paradoxical embolism of dense fibrous tissue following pacemaker lead extraction: salvation by mechanical thrombectomy

Systemic embolization is a dreaded complication of transvenous lead extraction (TLE), even without visible vegetations. Preoperative patent foramen ovale evaluation is important, justifying neurological surveillance or consideration of surgical extraction in selected cases. In case of stroke after TLE, mechanical thrombectomy is a successful therapy, and should be readily available.

Accessory atrioventricular pathways refractory to catheter ablation: role of percutaneous epicardial approach.

BACKGROUND Epicardial mapping and ablation of accessory pathways through a subxiphoid approach can be an alternative when endocardial or epicardial transvenous mapping has failed. METHODS AND RESULTS We reviewed acute and long-term follow-up of 21 patients (14 males) referred for percutaneous epicardial accessory pathway ablation. There was a median of 2 previous failed procedures. All patien...

Unexpected challenging case of coronary sinus lead extraction

An 84-year-old woman implanted with cardiac resynchronization therapy defibrillator underwent transvenous lead extraction 4 mo after the implant due to pocket infection. Atrial and right ventricular leads were easily extracted, while the attempt to remove the coronary sinus (CS) lead was unsuccessful. A few weeks later a new extraction procedure was performed in our center. A stepwise approach ...

Successful stent implantation for superior vena cava injury during transvenous lead extraction

Introduction The increased number of implantations of pacemaker and implantable cardioverter-defibrillator (ICD) devices has also generated a rise in the number of lead extraction procedures, with an estimated 10,000 to 15,000 devices extracted annually worldwide. The difficulty of lead extraction is directly proportional to the severity of the fibrotic and adhesive scar tissue that encases the...
